trivium (n.) - A Crossroads where three roads meet.

At Trivium Wellness, we believe that health is a journey, and sometimes, we all reach a crossroad.  That’s where we meet you.  Our clinicians guide you toward the optimized path to movement and wellness through compassionate care, clinical expertise, and genuine partnership.

​

Trivium was founded by Dr. Alexandra Gehlen, PT, DPT, who envisioned a practice built on the harmony of mind, body, and spirit.  What began as her personal mission to meet every patient at their healthcare crossroad has evolved into a collective of Central Oregon’s most skilled and passionate physical therapists, each brining their own expertise, perspective, and dedication to empower our community.

​Together, our clinicians share a simple goal: to help you feel your best, move with confidence, and get back to doing what you love, efficiently and effectively.

Dr. Alexandra Gehlen PT, DPT

​Alex Gehlen, DPT, is a dedicated physical therapist and the founder of Trivium Wellness in Bend, Oregon.
 

Originally from the Midwest, Alex earned her Doctor of Physical Therapy degree from Saint Louis University, whose program is ranked among the top 10% nationally. Before establishing Trivium Wellness in 2022, she spent three years as a traveling physical therapist, gaining diverse clinical experience across various settings nationwide.

​

After moving to Bend, Alex served as the Director at Step and Spine Clinic, where she deepened her expertise in treating complex, chronic conditions. Her work there inspired her to branch out and create Trivium Wellness, a practice dedicated to providing personalized, movement-focused care that empowers individuals to achieve their goals.

​

Alex specializes in spinal, pelvic, and hip instability, offering a biomechanical approach to treatment that optimizes movement, alleviates pain, and helps patients return to the activities they love.

 

Dr. Gehlen has become a trusted resource among local physicians for successfully treating individuals with chronic conditions that have not progressed at other clinics.

​

Outside the clinic, Alex embraces Bend’s active lifestyle. You’ll often find her snowboarding at Mt. Bachelor, mountain biking at Phil’s Trailhead, or enjoying a local coffee shop.

Dr. Samantha Noregaard PT, DPT

Sam Noregaard, DPT, is a passionate physical therapist at Trivium Wellness in Bend, Oregon.
 

Sam specializes in women's pelvic health and orthopedic physical therapy, helping women understand, heal, and strength their bodies at every stage of life.  Whether you're navigating pregnancy, recovery postpartum, or managing pain, leaking, or pelvic pressure, Sam helps you identify what's really happening, and gives you the tools to feel confident and in control again

​

She has completed advanced training through Herman & Wallace (Pelvic Floor Level 1 & 2A) and is finalizing her Certificate of Advanced Practice in Physical Therapy in Obstetric Health, one of the highest distributions in women's pelvic therapy.

​

Additionally, Sam is passionate about treating patients with spinal pain, pelvic pain, and lower extremity injuries.  She enjoys working with individuals returning to running, working to prevent injury, and rehabbing post operative cases.  

​

Sam's calm, educational style helps patients feel seen, supported, and empowered through every step of care.

Dr. Matt Carter PT, DPT

Matt Carter, DPT, is a seasoned physical therapist at Trivium Wellness in Bend, Oregon.
 

Matt is known for helping people overcome chronic neck, shoulder, and spine pain, especially when the problem has lingered or resisted other treatments.  

​

Matt takes a full-body approach to uncover how subtle movement patterns or past injuries contribute to ongoing pain and stiffness.  Patients value his ability to quickly identify the source of discomfort and create lasting results using a mix of hands-on care, movement retraining, and simple education that puts them in control of their recovery.

​

Matt's philosophy is simple: movement is medicine.

​

His goal is always to help people feel and move better, efficiently, confidently, and with long-term results.

Leigh "Byrd" Graciano

Certified Personal Trainer | Postpartum Core Corrective Exercise Specialist | Integrated Health Specialist

​

Leigh’s passion for movement and well-being started early, at just nine years old with her first Barbie workout set (she has the picture to prove it!). That early love for fitness eventually led her to pursue a degree in Psychology and a career in the educational field, where she worked with developmentally delayed children in an occupational and physical therapy clinic.

​

After years of homeschooling her two boys and craving deeper community connection, Leigh combined her love for teaching and movement by becoming a certified personal trainer. She quickly grew into one-on-one coaching and group fitness instruction, later specializing in pre and postpartum fitness as a Postpartum Core Corrective Exercise Specialist.

​

As an Integrated Health Specialist, Leigh takes a holistic approach to movement and wellness, ensuring that each client receives personalized, functional training that aligns with their body’s unique needs. This credential deepens her ability to support clients through physical transitions, injury prevention, and long-term movement optimization, whether in the postpartum period, returning from injury, or striving for overall well-being.

​

Her training philosophy is simple: honor the body where it’s at and guide it toward sustainable strength and resilience.

​

When she’s not coaching or homeschooling, you’ll find Leigh outdoors with her husband and two boys, embracing all the adventure Bend has to offer.
